Should any medication be required for your child, and staff at Prodigy are (potentially) required to administer this, then a medication form must be filled out before the parent/carer leave site. 
This is to ensure we are happy with what has been left, and all questions have been answered. You can prefill this form before arriving to camp (find form below), alternatively we will have spares on site.
